If you would like to be considered to be a regular vendor for Sunrise Management, or notified the next time we put out a bid for a job, please fill out the following form. Submission of this information is not a guarantee of work.

If you are selected, you will be invited to begin our vendor validation process. We contract with a company, Compliance Depot, which qualifies prospective vendors, independent contractors, business partners, customers, etc. They will ask for a W-9 (download PDF file) and proof of insurance (sent directly from your insurance provider) verifying that your company meets the following criteria:

  • Vendors must have General Liability insurance with a minimum $1,000,000 coverage (additional insured listing Sunrise Mgmt. Co.),

    AND

  • Workers Compensation proof of coverage (additional insured listing Sunrise Mgmt. Co.).
